Failed to Save. Could not find the custom XML part to delete.

I created a docx file with content controls and mapped them using the toolkit. Worked great. Closed all files, reopen using toolkit and changes some of the text in one of the sections. Tried to save and got the error dialog "Failed to Save. Could not find the custom XML part to delete."

The xml I entered was very simple. The only change I made was to the <paragraph2> element, I changed the text from P2 to P2 - new.

<Paragraph1>

P1.

</Paragraph1>

<Paragraph2>

P2 - new

</Paragraph2>

<Paragraph3>

P3.

</Paragraph3>

I wasn't able to reproduce this issue on my local test.

Here's exactly what I did:

1. Boot Word
2. Add 3 plain text content controls to the document body
3. Save the document
4. Close Word
5. Boot the Content Control Toolkit
6. Open the document
7. Create a new XML part
8. Change to Edit View and paste in the following XML:

<Paragraph1>

P1.

</Paragraph1>

<Paragraph2>

P2.

</Paragraph2>

<Paragraph3>

P3.

</Paragraph3>

</root>

9. Click Bind View
10. Dragged each paragraph node to the corresponding content control to bind them
11. Save and close the toolkit
12. Boot Word and open the document
13. After verifying it wored, close the document and Word
14. Boot the toolkit, and reopened the document
15. Change the text of Paragraph2 to P2 - new
16. Clicked save.
17. Close the toolkit
18. Opened the document in Word to verify everything worked

Can you give me a detailed list of steps like I provided so I can debug this?
